Step 1 — Check out the brochure

Step 2 — Fill out the registration form by May 2

Turn in your completed registration form and payment by doing one of the following:

• Drop it off at Blackhawk
  During office hours: Main office
  On Sunday: Work room (#212) in the Middle School Ministry box
• Mail it to:
  Blackhawk Church
  Attn:  MS Madison Missions
  9620 Brader Way,  Middleton, WI  53562
• Bring it to Home Group or First Wednesday

Step 3 — Turn in your payment

• Check payable to Blackhawk Church
• Blackhawk students click here to pay online (online payment is not available for Asbury and High Point — sorry for any inconvenience)
